Anyone participating in an experience or audition camp will need to submit the Medical Release Form, and Release of Liability Form. They must be signed, dated, and completely filled out. Anyone under the age of 18 years old (minors) must have these signed and dated by a parent or guardian. You may bring the forms with you, or you may scan them and submit them to: documentation@resistanceindoor.org. Anyone that shows up without these completed as described, will not be able to participate. Anyone issued a callback from a virtual audition will need these forms for the callback camp. Please feel free to contact us with any questions you may have.
